Yes, I used the "1.18PCIDV" that I download from "http://welcome.to/3dfxbios" too.
Instructions are at http://welcome.to/3dfxbios as well.
DVI works great, but you can't use both the DVI and the SVGA at the same time (i.e. 2 monitors).
Put the monitor on the AGP card first so you can see what you are doing when you flash.
After you get the "flash successful" message, put the monitor on the PCI card and remove the AGP card. Simple!

Um, yes and just don't worry about the problems I had in that thread PS47 has referenced - that card was DEAD so that was the real problem.
I purchased another MAC PCI Voodoo 5 5500 the other day and it flashed fine - even the DVI works! You just have to ensure you use the version 1.18 BIOS.
I placed that vard in a Abit BE6 mobo with a PIII-500 running XP and the AmigaMerlin 3.1 drivers and I'm getting 40-60fps on Unreal Tournament!
Also, don't worry too much if the flash program locks up half way through programming the EEPROM - just reboot the PC and try again - it really is failsafe.
