I have resolved the missing checkbox problem, it only appears when using NTSC, so if it´s missing you need to reboot the PC using TV-out and have NTSC selected. I assume the the monitor is blank, so you need to work on your TV [ or use "Alt-F4" and do it blind]
I´ve only tried this route, could work rebooting it witout TV-out selected, but make sure NTSC is selected.
The reson for all this is related to your second problem, the weird monitor display.
So far my theory is this:
The TV-out and the VGA-out are using the same source, they are not independant of eachother. So the signal sent to the TV is the same displayed on the monitor. For that reson monitor displays a stretched image and the VooDoo card refuses you to use dual display in PAL-mode since it assumes the monitor can´t handle it (most can´t so it´s just making things idot proof)
Hope you understand what I´m getting at.
p.s.
My last hope is that I´m wrong, that there is a way of having PAL out on the TVoutput but still have a outpout to the VGA that it can handle.
One way out is two Video-cards
