From all the product images, chip photos and dead end link photos, my best quess is this could be a Voodoo 4 4500, but I really need to be sure, because it's going into my wife's new PC and I don't want to blue screen it! It says "N2544" and "210-0416-001-AO" printed near the chip. Since the cooler is glued onto it, I don't want to tamper with prying it off. The other side has a sticker that seems to be partially missing, but reads "V4454, then looks like part of a 3".
